Scope of the Role:
The Social Worker reports to the Executive Director and is responsible for providing orientation, informal education, support and counselling to residents, families, and friends to promote a sense of comfort and well-being, as well as overall empowerment among individuals. Intervenes as needed in interactions between individuals, families, staff, group communities and their environment.
Responsibilities:
- Develop, implement, and evaluate social service programs.
- Support department managers with staff interventions.
- Facilitate support groups and provide bereavement counseling for residents, families, and staff.
- Coordinate Family Council meetings.
- Maintain communication with placement agencies, applicants, and representatives to meet occupancy targets.
- Conduct outreach with hospitals, community agencies, physician offices, and other referral sources.
- Understand, monitor, and help improve quality indicators.
- Participate in recruitment, orientation, retention, labor management, and performance management.
Review resident satisfaction data monthly and quarterly; develop action plans, with semi-annual analysis reported to the Executive Director. - Perform other duties as assigned per Job Task Inventory.
Qualifications:
- Diploma in Social Services, Gerontology, or a degree in Social Work.
- Registered under the Social Work and Social Services Work Act with a current general certificate, or three years full-time (or equivalent part-time) experience in a similar LTC role within the past 5–7 years.
- Experience in geriatric social services is an asset.
- Strong interpersonal, organizational, written, and verbal communication skills; able to interact courteously with residents, staff, volunteers, and the public.
- Knowledge of RAI-MDS – AIS certified, or must complete within one year of hire.
- Demonstrates professional social work values: respect for individual dignity, autonomy, and rights; supports personal growth; understands individuals within interdependent systems; provides assistance and protection when needed.
Compliance & Health Requirements:
- A current Vulnerable Sector Check (including a Criminal Background Check)
- completed within the past six months, or willingness to obtain one.
- TB test results dated within the last six months, or within 14 days of hire; alternatively, a chest X-ray completed within the last year, in accordance with Public Health guidelines.
- Proof of all required vaccinations must be provided. Staying "up to date" with recommended vaccines is strongly encouraged. Requirements are subject to change based on Employer directives.
