Responsibilities of the Documentation Nurse at Ina Grafton Gage Home

Is responsible for the overall day-to-day delivery of health care operations, ensuring that the health care services comply with the Fixing long Term Care Act (FLTCA) and other relevant legislation.

Essential Duties are the technical skills and knowledge that an employee requires in order to perform their role. Below is detailed information on required Essential Duties for your position.

Responsibilities and Duties:

• Development and modification of the plans of care in response to residents’ individualized health care needs and change in health care status

Completes health care assessments (RAI-MDS and other assessments), develops and updates plan of care for each resident in collaboration with the resident and/or SDM and the interdisciplinary team.

Liaises with other health care personnel, agencies and families

Maintains good relationships with Medical/Support/Home staff, residents, families and general public
Supervises all activities on the unit, takes charge as required

• Assists with the MDS Schedules, documentation  and monitoring of all informatics on residents assessments

• May be required to work on the unit as required

• Participates in quality initiatives resulting as directed

  Conducts unit rounds at the beginning and end of shift. 

 Processes Physician orders and administers medications and treatments in compliance with the policies of Pharmacy provider, the Home and the Standard of the College of Nurses. Documents and reports all errors and omissions 

 Maintains accurate and complete records of nursing observations and care in compliance with the legislative requirements and the policies of the Home 

 Assesses changes in health status and report  

Qualifications:

• Registered health professional licensed under the Health Disciplines Act

• Well-developed written and oral communication skills

• Knowledge and experience with adult education strategies/facilitation

• Proficient computer skills and strong problem solving abilities

• Ability to work with and relate to interdisciplinary team members and other internal stakeholders

• Knowledge of RAI-MDS – AIS certified, if not must be completed successfully within one (1)year of being hired

• Skills and knowledge in trending and analyzing data

• Knowledge of Quality Improvement * Minimum of two (2) years experience in a long term care setting

Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one

  • As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements
  • Two supervisory references required
